Our initial consultation is always free. During the initial consultation, we will listen to your concerns, offer our initial impression of your situation, and determine how we can be of assistance to you. We will discuss our recommendations for moving forward. If we do not believe our services are right for you, we will refer you to other professionals or organizations that we believe may better serve your needs.
We offer the following Aging Life Care services.
Structured for elders that have support or are able to handle most of their own care needs, but who need guidance and assistance in formulating a care plan or accessing specific services. Our Consulting begins with an In-Home Initial Assessment from which a comprehensive and individualized Care Plan is prepared. The Care Plan is then reviewed with the elder and/or their own personal care coordinator. After that, Peninsula Care Consultants remains available on an “as needed” basis for further clarification or guidance as circumstances may change. We’re only a phone call away.
Structured for elders that need ongoing support or have more complex circumstances and needs, and elders with family members living out of the area or otherwise unable to assist with care coordination. Our Care Management begins with an In-Home Initial Assessment from which a comprehensive and individualized Care Plan is prepared. The Care Plan is then reviewed with the elder and/or their family members. Peninsula Care Consultants then follows the agreed upon Care Plan in building a support system for our client. Usually Care Management is most labor intensive at the very beginning. Once our clients have a safe and supportive care system in place, Peninsula Care Consultants will continue to make in-home visits at the agreed upon intervals, depending on needs, and will report back to family members as applicable so that you can rest easy knowing your elder is safe, that they have support, and that they are not alone.
Family Solutions Care Conferences
We are available for Family Solutions Care Conferences when the entire family needs to come together to support a loved one. During this process, we focus on the medical status of the elder who will be receiving care. We focus on the elder’s preferences, their quality of life issues, and any barriers to care or quality of life that may be present. We then ask all family members for their feedback so that everyone’s voice is heard and all concerns are presented. Together, we work out a Care Plan that is then presented to the family for review and acceptance.
